Caprini Score for Varicose Vein Surgery

Varicose Vein Surgery VTE Risk Calculator (CAPSIVS Protocol)

The Caprini Score is the most widely used VTE risk stratification tool in surgical patients. This calculator is the varicose vein surgery variant, with the set of risk factors and management specific to the phlebological population, drawn from the work of the Lobastov group and the CAPSIVS registry, published in 2023 and reviewed in 2024.

For colleagues who want to use the tool and carry the result over into their patients’ electronic medical record: once the score is calculated, an export button appears that produces a plain-text file (.txt), which can be saved or copied and pasted into the chart. Printing produces a one-page record sheet with the same content as the .txt file.

How to use it

  • Fill in the patient details — name, record number and age. Age is the only required field: it selects the age band and scores automatically.
  • Tick every factor that applies. The score updates live, so you do not need to calculate anything while filling in the form.
  • Click “Calculate score” to see the result, the risk stratum and the recommended management.

Where the cut-offs on this scale come from. The 0–4, 5–8 and ≥ 9 cut-offs do not come from CAPSIVS: they are the general-surgery Caprini convention, kept here for compatibility with the previous version of this page. The CAPSIVS registry (Lobastov 2023) analysed the score continuously and found no clear cut-off; the 2024 review proposes a different cut-off for the phlebological population — 0–6 low, 7–10 moderate and ≥ 11 high. The three readings are not interchangeable: confirm which convention your service follows before using the result to guide management.

Why “Very high” does not depend on the score. The trigger is limited to a personal history of VTE and to personal or family thrombophilia. The 2024 review attributes to patients with a personal history of VTE a roughly 4.2-fold higher risk of thrombosis, with secondary prophylaxis for at least 30 days — and, in selected cases, for 3 to 6 months. A family history of thrombosis on its own does not extend prophylaxis: it still counts as 3 points like any other Caprini item and does not trigger the top band.

On the duration of prophylaxis. The source supports pharmacological prophylaxis in the moderate and high strata, with a minimum duration of 7 days in the moderate group and up to 30 days in the high one. The VTE risk after minimally invasive procedures on superficial veins reaches 3.4%, of which only 0.5% are symptomatic; the risk rises sharply when varicose tributaries are removed at the same time as perforator veins are treated.

A support tool, not a prescription. This calculator does not assess bleeding risk or contraindications. Final management must be determined by the attending physician, taking into account the particularities of each patient, the type of procedure performed and the institutional protocol.

How this model differs from the general Caprini score

Three points deserve emphasis, because they do not follow the reading of the general Caprini calculator:

The "Very high" stratum does not depend on the score. A family history of thrombosis on its own does not extend prophylaxis: it still counts as 3 points like any other Caprini item, but it does not trigger the top band.
Planned surgery is cumulative. Laser, saphenectomy and associated phlebectomy add up to one another; they are not alternatives.
Age is scored from the number you type. There is no group of options to tick: the value entered sets the age band and the points it carries.

Mind the stratum cut-offs. The scale in this calculator uses 0–4 low, 5–8 moderate and ≥ 9 high. Those cut-offs do not come from CAPSIVS — they are the general-surgery Caprini convention, kept here for compatibility with the previous version of this page.

Clinical note

This calculator is a support tool for clinical decision-making in varicose vein surgery. Final management must be determined by the attending physician, taking each patient's particularities into account.
Warning: This assessment does not replace clinical judgement. This calculator does not assess bleeding risk or contraindications — confirm both before prescribing pharmacological prophylaxis. The type of procedure performed (removal of varicose tributaries, treatment of perforator veins) markedly changes the risk, and follow-up with Doppler ultrasound is part of management in the higher strata.
